Apply For An Original Disabled Parking Placard or License Plate

• Submit your request for disabled parking placards or plates using either of the following methods:
  • By mail:

  • In person at any DMV customer service center.

• Submit a completed Disabled Parking Plates or Placard Application, MED 10.

• Your application for a temporary or permanent disabled parking placard or disabled parking license plates must include your physician's, physician assistant's, podiatrist's, chiropractor's or nurse practitioner's certification of your disability. Podiatrists and chiropractors may only certify for the conditions or disabilities listed in the certification section provided for podiatrists and chiropractors on the MED 10 form. Refer to the Certification table on the MED 10 to determine the medical professional authorized to certify your allowable condition. If you are applying for a license plate showing the International Symbol of Access, and you already hold a permanent placard, a physician's, physician assistant's, podiatrist's, chiropractor's or nurse practitioner's certification is not required. If you hold disabled parking license plates and you are applying for a permanent placard, no physician's, physician assistant's, podiatrists, chiropractors or nurse practitioner's certification is required.

• If you are applying for a disabled veteran's license plate with the International Symbol of Access, present a MED 10 form and a Veteran's Certificate of Disability, VSA 54 signed by the Veterans Administration. Veterans with a qualifying disability are exempt from annual registration and are entitled to plates for one motor vehicle that is owned and used personally by the veteran.

• Organizations must present an Institutional/Organizational Disabled Parking Plates or Placard Application MED 11. This application must include a certification that the vehicle is registered in the name of the institution/organization and used routinely to transport citizens with disabilities.

• Temporary and permanent placards for citizens cost $5. With the required certification, a person may obtain the International Symbol of Access on a plate at no additional charge.
