Insurance Industry Services - Forms

Effective January 1, 2008, Virginia law requires any person convicted of violation of the provisions the Code of Virginia § 18.2-51.4, § 18.2-266 and § 18.2-272 to purchase a liability insurance policy with coverage that is double the minimum limits. Therefore, the insured's liability limits must be at least $50,000 for injury or death to one person, $100,000 for injury or death of two or more people, and $40,000 for property damage.

Insurers use forms FR-44 and FR-46 to submit financial responsibility information to DMV. Form FR-44 is used to certify that such coverage was purchased and in effect and form FR-46 is submitted to report cancellation of such coverage.
